Before Your Appointment.

Please bring a list of all the medications and supplements you currently use. This includes:

1. Prescription medication 

2. Non-prescription medicine such as aspirin or antacids

3. Vitamins, dietary or herbal supplements

During Your Appointment.

At your first appointment, please explain your symptoms, health history and any problems with medicines you have taken in the past in as much detail as possible.

Never hesitate to ask questions during your appointment. This includes clarification around treatment instructions, whether it’s possible to have a test done, or when you will receive the results of a test.

After Your Appointment.

If you have any questions regarding your treatment, please do not hesitate to call the clinic or write down your questions so you can discuss them at your next appointment.

Remember to always follow the instructions for your treatment plan.

Write Down Your Issues

Write down all of issues you would like to discuss during your visit and include as much detail as you can.

 

Ask for Clarification

Medical terminology can often be confusing. If you are unsure or not clear about anything that is said during your appointment, please don’t hesitate to ask for clarification.

Make a List of Medications

Bring a list of all current medications you are taking including supplements, over the counter medications and prescription medications.

 

Take Notes

If it’s helpful, please feel free to take notes during your appointment to reference later.

Share Info

If you feel that you have any information or research that may be helpful to your health condition, please do not hesitate to share this during your appointment.

 

Bring Support

If needed, please feel free to bring a friend, family member or an advocate with you to ensure you feel comfortable during your appointment.
