The nerve fibres that carry pain signals to and from the brain are thought to be damaged in postherpetic neuralgia. When the varicella-zoster virus infects a person, it can harm nerve fibres, resulting in inflammation and scarring. By interfering with the normal function of nerve fibres, this injury may cause the brain to receive abnormal pain signals.

Other factors such as an impaired immune system, advanced age and a severe episode of shingles can also contribute to the likelihood of developing postherpetic neuralgia.
